The Cook County Pension Fund (CCPF) has hired Concentra Occupational Health to complete the County Physician Statement for CCPF members applying for ordinary disability benefits. With a streamlined evaluation process and locations across Chicago and its surrounding suburbs, Concentra offers members greater efficiency and enhanced convenience.

Applying for Ordinary Disability

Members applying for ordinary disability must follow the application process outlined in the Disability Benefits Overview. Upon receipt of your required documentation, a member of the CCPF Disability department will contact you with the Concentra location you must visit for your evaluation. No appointment is needed, and CCPF staff will do its best to identify a Concentra location close to you. Please note: Concentra doctors will not be able to evaluate your case without the information CCPF provides about your case; to save time and ensure you visit the correct location, please wait to hear from us.
