The Cook County Pension Fund (CCPF) has mailed 1095-B forms to members who had eligible medical coverage in 2023. If you have not received your 1095-B by March1, 2024, please contact the CCPF office at 312-603-1200 for a reprinted form.

Please note: If you have moved, you must supply us with a completed Change of Address form prior to any reissue of your 1095-B.
