The Cook County Pension Fund mailed 1099-R’s for annuitants on January 20, 2021. If you have not receive your 1099-R by February 10, 2021, please contact the Fund office at 312-603-1200 for a reprinted form.

Please note: If you have moved, you must supply us with a completed Tax Center Change of Address form prior to any reissue of your 1099-R.
