All 1099-R tax forms were delivered to the US Post Office on January 26, 2016. If you do not receive your 1099-R by February 12, 2016, please contact our office for a reprinted form. You may request a reprint of your 1099R by calling (312) 603-1200, by emailing info@countypension.com, or by using the Contact Us link on this website. 

If you moved or have a new address, you must sign and complete a Tax Center Change of Address Form as soon as possible. Please download the form and mail or fax the form to the Fund office.

The amount in Box 5 on your 1099R form represents the employee contribution or non-taxable amount you received in 2015.  It is the difference between your gross amount (Box 1) and the taxable amount (Box 2a). The amount in Box 5 is NOT related to any health insurance deduction.
