On January 19, 2012 all 1099R’s were mailed If you do not receive your 1099R by February 6, 2012 please contact the Fund office for a reprinted form. Please Note: If you have moved you must supply us with an address change prior to any reissue of a 1099R.

The Fund will not reprint forms before February 6, 2012.
The amount in Box 5 on your 1099R form represents the employee contribution or non-taxable amount you received in 2011. It is the difference between your gross amount (Box 1) and the taxable amount (Box 2a). The amount in Box 5 is NOT related to any health insurance deduction.
